Holds may be granted for medical reasons (doctor note may be required) and must be requested in writing (form below) within 3 days of the incident.
Holds *may* be granted for personal reasons. These holds can only begin at the start of a new monthly cycle, not in the middle of one. See FAQs for more details.
It is the responsibility of the member to notify admin of their return date if it is different from the date written on the request form. A minimum of 2 business days notice is required.
Extension requests will be granted on a case by case basis.

Your renewal date can be found in ZenPlanner by clicking the 3 dots in the top left corner and then clicking on “view profile”. Please have this date handy.

If hold reason is “taking a short break” the hold MUST start at the beginning of a new cycle. Elective holds cannot start mid-cycle.
* Enter an estimated date of return. This date can be adjusted (for illness/injury holds ONLY) as needed by emailing admin ([email protected]) with sufficient notice.

If date needs to be extended, or you wish to return sooner than planned, please notify admin at least 2 days before the date entered on this form.

Extension requests are not guaranteed and will be assessed on a case by case basis.
